This Order for Service form is the acceptance of the quote provided by Transportation Nation. All information is completely confidential and secure. By completing this form, you accept the terms & conditions (below) and authorize Transportation Nation to charge a deposit only.
Terms & Conditions Transportation Nation reserves the right to charge extra fo rany items shipped which listed on the given quote. Transportation Nation schedules all pickup's and deliveries by reservation only. Transportation Nation does not guarantee pickup or delivery dates (unless specified). Transportation Nation gives "approximate" dates only. Transportation Nation charges a 10% cancelation fee if scheduled for pickup. Transportation Nation does not provide disassembly or reassembly services with the exception of pianos and organs (simple and basic assembly can be performed upon instruction by the customer). Transportation Nation is not responsbile for contents of packed by owner, or "PBO" items shipped.
Unusual Circumstances at Pickup or Delivery I have informed Transportation Nation of all circumstances involved in the shipment locations such as, but not limited to: stairs, elevators, long distance carries, etc. I understand that if there are stairs, elevators, exceptional long distance carries, gravel roads, mountain, hillside, or possible difficult or hazardous conditions at pickup or delivery, not stated in the original quote, I agree to additional charges to perform and complete the shipment properly.
